ABSTRACTS

Active participants are kindly requested to complete the attached Abstract Form in English and return it together with the diskette or CD-ROM and 3 printed copies to the address: IKEM, Department of Cardiology, Karel Roztočil, M.D., Vídeňská 1958, 140 21 Prague 4, Czech Republic - before December 31, 2003. Please, see the instructions on the Abstract Form. The abstracts can be sent also by e-mail: karel.roztocil@medicon.cz.

TECHNICAL EQUIPMENT

Data projection with presentation on diskette or CD-ROM as well as VHS video projection and overhead projection will be available.

EXHIBITION

The medical and technical exhibition of drugs, instruments and equipment will be located in the hall close to the conference hall on February 19 – 21, 2004.

REGISTRATION

All participants are requested to complete the enclosed Registration and Accommodation Form and to return it together with confirmation of payment to the Secretariat. Registration fees are indicated on the form. It is also possible to register on-line – see: www.angiologie.cz.

Registration fee includes: scientific programme and exhibition, conference materials, coffee breaks.

CANCELLATION OF REGISTRATION

The payment will be refunded minus 20% cover administration costs, if the registration is cancelled in writing before January 16, 2004. No refund will be possible after this date but the Secretariat will send the conference documents to the participant.

BOARD

Lunches can be ordered in advance on the Registration and Accommodation Form. They will be served in the Olympik Hotel's Ikarus restaurant.

ACCOMMODATION

Hotel or hostel accommodation can be arranged by the Secretariat on request. Please make your reservation preferably before January 16, 2004. Accommodation cannot be made without the deposit covering 50% of the amount due or 100% if your reservation is one night only. The hotel voucher will be mailed to you upon receipt of payment.

Conference venue. The hotels are situated on metro line B, 10 minutes from the historical centre of the city and have their own large guarded car-park (100,- CZK per day). The hotel rooms are fitted with modern furniture and equipment.

Hotel Čechie*** (incl. breakfast), U Sluncové 618, Prague 8 - Karlín
The hotel is situated near the conference venue, 10 minutes by metro from the historical centre of the city. Price includes free entry to swimming pool and sauna. Parking in the garage (250,- CZK per day) or free in front of the hotel.

Novoměstský Hotel*** (incl. breakfast), Řeznická 4, Prague 1- Nové Město
This hotel is located in the city, close to shopping and entertainment. It takes way about 10 minutes to reach the venue by metro line B from Karlovo náměstí to Invalidovna.

Masarykova Kolej *** (incl. breakfast), Thákurova 1, Prague 6 - Dejvice
Student hostel - nice accommodation of a standard similar to *** hotel. The way to the venue: five minutes to the metro line A Dejvická station, then change to line B at Můstek and go to Invalidovna. Guarded car-park (50,- CZK per day).

Hotel Aura *** (incl. breakfast), Kostelecká 819, Prague 9 - Čakovice
Newly renovated hotel in a quiet part of Prague, near Aquapark and the Globus and Tesco shopping centres. The way to the venue is by bus no. 133 or 140 to the metro line B Palmovka station (15 minutes), then 5 minutes on foot or by metro or tram to Invalidovna.

Kolej Petrská** (incl. breakfast), Petrská 3, Prague 1
Student hostel, near the Florenc bus station and Masarykovo nádraží railway station. Easy and quick access to the conference venue by metro or tram.

Kolej Větrník* (without breakfast), Na Větrníku 1932, Prague 6 – Petřiny
Newly reconstructed hostel - simple and cheap accommodation.
The way to the venue:
-by tram no. 1 to the Palmovka stop, then 5 minutes on foot or one stop by tram
-by bus no. 191 to the Anděl stop, and from there by metro line B to Invalidovna.

Bubenečská Kolej* (without breakfast), Terronská 28, Prague 6 - Dejvice
Student hostel - very simple and cheap accommodation, five minutes from the metro line A Dejvická, then change to line B station at Můstek, and go to Invalidovna.
