All participants are requested to complete the enclosed Registration Form and to return it together with the payment confirmation to the secretariat. Registration fees are indicated on the form. It is possible to register on-line at the internet address: www.angiologie.cz
Registration fee includes: participation in the scientific programme and exhibition, congress documentation, coffee breaks, Get-together party.

The payment will be refunded minus 20% cover administrative costs, if the registration is cancelled in writing form before February 28, 2003. No refund will be possible after this date but the secretariat will send the congress documents to the participant.
Active participants are kindly requested to complete the attached Abstract Form in English ant return it together with the diskette (in Microsoft Word precessing programme) and 3 printed copies to the address: IKEM, Department of Cardiology, Videnska 1958, 140 21 Prague 4, Czech Republic - before January 31, 2003. Please, see the instructions on the Abstract Form. The abstracts can be sent also by e-mail: karel.roztocil@medicon.cz.
Data projection with presentation in Power Point at the diskette or CD as well as video VHS projection and overhead projection will be available. It will be not possible to present your lecture on slide projection!!!
Coffee and tea will be served during the breaks twice a day. Lunches will be available in the restaurant at the meeting venue in the charge of participants 5 EUR / lunch.
The medical and technical exhibition of drugs, instruments and equipment will be located in the gallery close to the conference hall on April 3 - 5.
